Job summary

South West London and St George's Mental Health NHS Trust is seeking an experienced Mental Health Pharmacist to lead medicines optimisation across community mental health teams. The role focuses on reducing hospital admissions and improving patient experience with prescribed medicines.

You will work within a multidisciplinary team across community services, with opportunities to develop Non-Medical Prescribing skills and influence service development in Richmond upon Thames.

Qualifications

Experienced Mental Health Pharmacist delivering medicines optimisation within community mental health teams.
Ability to work within a multidisciplinary team to improve patient outcomes.
Opportunities to utilise Non-Medical Prescribing skills and provide expert guidance.

Responsibilities

Deliver specialist medicines optimisation services to community mental health teams.
Collaborate with psychiatrists, prescribers, GPs and the wider MDT to optimise treatment.
Undertake medication reviews and make recommendations on initiation, optimisation, switching or discontinuation.
