Job summary

Berkshire Healthcare NHS Foundation Trust is seeking an Advanced Specialist Mental Health Pharmacist to lead medicines optimisation across CPE within BHFT's mental health and community services.

The post holder will review referrals, provide expert medication advice to GPs and primary care colleagues, and develop prescribing policies and systems in collaboration with the Clinical Pharmacy Services Lead. Research with university partners is encouraged.

Qualifications

• Masters in pharmacy or equivalent from a British School of pharmacy or recognised equivalent.
• Statutory registration with GPhC.
• Clinical or psychiatric postgraduate certificate.
• Clinical or psychiatric postgraduate diploma.
• Independent prescriber.

Responsibilities

• Develop, plan, manage and lead a high quality medicines optimisation service to CPE and BHFT.
• Provide expert, specialist medication referral point for healthcare professionals in CPE as a core MDT member.
• Advise on medication and physical health improvement needs for individuals referred to the service.
• Collaborate with Local Mental Health Services and inpatient pharmacists to ensure seamless pharmaceutical care.
• Liaise with community pharmacists and GP practices to ensure appropriate supplies and monitoring.
• Directly liaise with GPs on prescribing/de-prescribing as appropriate.
