Central London Community Healthcare NHS Trust is seeking a Senior Pharmacist (Band 8a) to strengthen the Medicines Management team across Community Hospitals in London. You will lead a dynamic team, develop services, and drive quality improvements while delivering patient-centered care.

The role offers NHS terms, with 37.5 hours per week, and requires extensive clinical pharmacy experience plus leadership skills to enhance medicines optimisation across Primary and Secondary care interfaces.

Qualificações

• Post-qualification experience of clinical pharmacy, medicines information and prescribing advice
• Experience of leading a team within a hospital or community setting
• Experience of service development and quality improvement in medicines management

Responsabilidades

• Support and deliver a high quality clinical pharmacy and medicines optimisation service to patients in Community Hospitals.
• Ensure an integrated approach to medicines management and continuity of pharmaceutical care for patients admitted from primary and secondary care into Community Hospitals.
• Contribute to the Trust’s clinical governance through interventions that optimise medicines management across all areas of practice.
• Lead, manage and develop staff within the Bedded Services team who work in Community Hospitals
