Responsabilités

• Support and deliver a high quality clinical pharmacy and medicines optimisation service to patients in Community Hospitals.
• Ensure an integrated approach to medicines management and continuity of pharmaceutical care for patients admitted from primary and secondary care into Community Hospitals.
• Contribute to the Trust's clinical governance through interventions that optimise medicines management across all relevant areas of practice.
• Lead, manage and develop staff within the Bedded Services team who work in Community Hospitals.
• The post holder will work as an active member of the Medicines Management team, contributing to the delivery of a safe, high quality and legally compliant service that aligns with national and local policies and priorities.
