Job summary

Kent Community Health NHS Foundation Trust is seeking an experienced Band 8a Senior Medicines Optimisation Pharmacist to join the Locality Medicines Optimisation Team. You will shape medicines optimisation across Primary Care Networks, GP practices and the wider Kent and Medway system to ensure safe, effective, evidence-based prescribing.

Responsibilities

• Lead and deliver a specialist pharmacy service to a directorate or community.
• Provide expert advice on pharmaceutical matters in a specialist field.
• Lead and develop clinical audit; undertake research.
• Provide support on defined Kent and Medway MO work streams to ensure safe and cost-effective use of medicines within clinical pathways.
• Undertake tasks as deemed appropriate by the line manager.
• Support the delivery of Medicines Optimisation projects in locality, including primary and secondary care.
• Prepare and deliver prescribing reports via face-to-face or online meetings.
• Provide medicines optimisation expertise and advice to the system-wide stakeholders including primary and secondary care healthcare teams.
• Support the portfolio of initiatives in demonstrating value for money for current spend through tracking, managing and delivering agreed
