Devon Partnership Trust's Liaison Psychiatry Service provides a 24/7 in and out of hours comprehensive service at the local District General Hospital, offering specialist mental health assessment and intervention for adults 18+.

The team delivers consultation-liaison support, education, and a range of psychosocial treatments, aiming to improve care pathways and patient outcomes in Torbay and South Devon DGH.

Mansioni

• Contribute to the team and engage with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.
• Impart complex and sensitive information to patients and families regarding assessment, care planning, treatment and review.
• Liaise with multi-disciplinary teams to ensure coordinated care and safe handovers.
• Plan and coordinate day-to-day workload with service user needs and wider service requirements.
• Undertake biospsycho-social assessments and develop formulations to identify onward care pathways.
• Participate in audit, research and service development activities within the team.
• Provide clinical supervision and mentorship to junior staff and students allocated to the team.
• Maintain up-to-date clinical records and ensure policy-compliant documentation.
