Job summary

Central London Community Healthcare seeks an experienced Learning Disabilities Clinical Assessor to join the Continuing Healthcare team in North West London. Ideal for an RNLD or experienced AHP dedicated to improving outcomes for people with learning disabilities.

You will assess, monitor and review individuals under the National Framework, working with service users, families and professionals to ensure safe, person-centred care and robust decision-making.

Qualifications

• Registered Learning Disabilities Nurse (RNLD) or suitably experienced Allied Health Professional.
• Experience with NHS Continuing Healthcare assessments.
• Experience in multidisciplinary settings.
• Ability to assess, monitor and review individuals within the National Framework.

Responsibilities

• Undertake NHS Continuing Healthcare assessments, reviews and monitoring visits.
• Apply the National Framework to support robust, evidence-based decision-making.
• Collaborate with multidisciplinary teams, provider organisations and families to achieve positive outcomes.
• Gather, analyse and present complex information from multiple sources.
• Support person-centred care planning and ensure services remain responsive to changing needs.
• Provide specialist advice on NHS Continuing Healthcare processes and eligibility.
• Contribute to safeguarding processes and risk management.
• Maintain accurate clinical records and documentation.
• Participate in service improvement initiatives and support high-quality Continuing Healthcare services.
• Balance a varied workload while working independently and as part of a wider team.
