Job summary

Essex County Council in Chelmsford is seeking an experienced Housing Occupational Therapist to lead major adaptations and support discharge planning across North/South Essex. You will assess housing needs, coordinate with Health and Housing colleagues, and guide multi-disciplinary teams to prevent delays in care.

You will mentor Health and Community OTs, manage complex cases, and drive evidence-based housing solutions for adults and children with life-changing conditions.

Qualifications

• HCPC registration and current professional practice in social care.
• Significant social care experience with knowledge of Care Act, Mental Health Act, and related guidance.
• Proven ability to deliver major property adaptations and housing solutions.
• Experience mentoring staff and acting as subject matter expert in practice.
• Experience chairing multi-agency meetings with families and professionals.
• Ability to build strong partner relationships and manage care package costs.
• Evidence of continuing professional development.

Responsibilities

• Complete holistic housing assessments considering all factors impacting the individual and household.
• Work with Health colleagues to process referrals for safe, early discharge and avoid delays.
• Refer to universal services and liaise with Housing Officers to meet assessed needs.
• Provide equipment and signpost to additional services where appropriate.
• Make recommendations for adaptations and equipment aligned with policy and legislation.
• Progress and support complex cases with MDT input when required.
• Monitor referral pathways and supply data to support cost-saving projections.
• Support Disabled Facilities Grant funded recommendations and ensure timely progress.
