Zusammenfassung

Essex County Council is seeking a qualified Occupational Therapist to join our housing and social care teams. The role focuses on delivering major adaptations, facilitating safe and timely discharges, and working with district councils and health colleagues.

You will mentor staff, chair meetings where needed, and liaise across multi-agency partners to meet complex housing and care needs. A valid HCPC registration and driving licence are essential.

Qualifikationen

• Qualified Occupational Therapist with HCPC registration.
• Significant experience in social care, knowledge of Care Act, Mental Health Act, Mental Capacity Act and related guidance.
• Experience delivering major adaptations.
• Experience mentoring staff and acting as a subject-matter expert.

Aufgaben

• Complete holistic assessments considering housing factors and household needs.
• Collaborate with Health colleagues to process referrals for safe discharge.
• Liaise with Family Operations to meet individual needs.
• Provide recommendations for adaptations and equipment based on assessed needs.
• Support and monitor referrals and pathway progress; provide data for cost savings.
• Work with Housing Officers to support DFG funded adaptations.
• Mentor Health and Community OTs involved in complex cases.
