This is an exciting role that will provide tailored Health & Wellbeing Coaching on a 1:1 basis with the option to deliver group support, to people living in Hammersmith & Fulham area. We work with the local primary care network to support clients who are looking to improve their Health & Wellbeing.

A typical day as a Health & Wellbeing Practitioner might include:

  • Delivering 1:1 sessions focused on behaviour change and lifestyle improvement, covering areas such as physical activity, sleep, mood, alcohol intake, and diet.
  • Managing a caseload of clients and monitoring and recording outcomes in Microsoft SharePoint and SystmOne.
  • Planning and delivering group sessions both face to face and remotely via MS Teams, including triaging participants to ensure suitability and engagement.
  • Building and maintaining strong working relationships with GPs, reception teams, social prescribers, and other healthcare professionals to encourage and promote referrals and to integrate the service within primary care.
  • Helping to promote the service externally by connecting with local community organisations and creating referral pathways.
  • Triaging referrals into the service, assessing eligibility and ensuring individuals are directed to the most appropriate support.
  • Motivating, inspiring and supporting individuals to manage their weight and adopt a healthier lifestyle, through a professional, knowledgeable and empathetic approach.
  • Ensuring an excellent and safe experience for clients, aligning with our internal policies and procedures.
  • Co-producing with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments; for example, sharing ideas for creating new resources or patient feedback pathways– we’re always looking to share knowledge and resources!
  • Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

To work with us in this role you will need to have some delivery experience of supporting clients to make healthy lifestyle choices. Full training for the role will be provided, however if you have already completed your NCSCT Stop Smoking Advisor training that’s a bonus!

Contract: We have 1 full time equivalent roles on offer (our full-time hours are 37.5 hours), permanent contract.

Salary: Starting at £27,000 FTE (rising through annual increments to £30,000). Please note, a pro rata salary is applied for part time positions.
