Somerset Council is seeking a Service Manager – Locality Health Interface to lead the countywide Health Interface Service, aligning Adult Social Care with NHS partners for safe hospital discharges and integrated care across Somerset.

You’ll oversee 33.41 FTE across senior social care roles, manage a ~£1.7 million staffing budget, and drive performance, safeguarding and service improvement through collaboration with NHS colleagues and voluntary sector partners.

Formación

• Senior management experience leading multidisciplinary teams.
• Experience working across health and social care with NHS partners.
• Knowledge of Adult Social Care legislation and safeguarding.
• Ability to use performance and financial data to drive improvement.
• Strong leadership, coaching and communication skills.

Responsabilidades

• Provide strategic and operational leadership to the Health Interface Service.
• Oversee performance, safeguarding, workforce and resources across Somerset.
• Collaborate with NHS colleagues, commissioners and voluntary partners.
