Job summary

Somerset Council is hiring a Hospital Interface Service social worker based at Yeovil District Hospital. You will join a fast-paced, multidisciplinary team, provide assessments, discharge planning and support to adults and carers throughout the hospital journey.

The role offers a permanent contract, 37 hours per week, and a starting salary of £38,510 rising to £43,149 per annum, with 30 days annual leave and ongoing professional development.

Qualifications

• Social Work qualification and current registration with Social Work England.
• Knowledge of adult social care practice, relevant legislation and issues affecting adults and carers.
• Experience working with multidisciplinary teams and partners.
• Ability to prioritise competing demands and maintain records.
• Empathy, sensitivity and resilience in fast-paced settings.
• Access to a vehicle for regular travel.

Responsibilities

• Undertake strengths-based assessments of risk, independence, care and support needs.
• Manage a varied caseload and respond to referrals and crises.
• Collaborate with health colleagues and partner agencies for admission avoidance and discharge planning.
• Complete carers’ assessments, care and support planning and records.
• Build effective relationships with adults, carers and multidisciplinary teams.
• Contribute to multi-agency decision-making and service improvement.
