Job summary

Somerset Council is hiring a Service Manager – Locality Health Interface to lead the countywide Health Interface Service, spanning hospital social work and associated governance. You’ll oversee performance, safeguarding and workforce, ensuring integrated care for adults in Somerset.

You’ll manage approximately 33.41 FTE across Advanced Practitioner, Adult Social Care Practitioner and Social Worker roles, with a staffing budget around £1.7 million, and travel across Somerset with occasional

• Strong understanding of Adult Social Care legislation and safeguarding

Responsibilities

• Provide strategic and operational leadership to the countywide Health Interface Service
• Ensure high-quality Adult Social Care support within acute hospital settings
• Lead performance, safeguarding, workforce, and resources
• Collaborate with NHS, commissioners and partners to improve services
• Develop and empower staff through workforce planning and coaching
