Job summary

Leicestershire County Council is seeking a HART Care & Support Worker to join the domiciliary care service. The role focuses on reablement, short-term crisis support, and enabling people to regain independence after hospital discharge.

You will work across county sites, on a rota including nights, weekends and bank holidays, with a DBS check required and driving license to travel between locations. The service emphasises accessible, inclusive care and collaboration with partners to meet each

Qualifications

• NVQ Level 2 in a relevant field or other equivalent qualification.
• Experience of working with other agencies and/or in a multi-agency environment to identify and meet need with creative and pragmatic solutions.
• Knowledge of working with people from all walks of life and delivering good quality services within allocated resources.

Responsibilities

• Provide home care and reablement support to help people regain independence.
• Support timely discharge from hospital and prevent admission to hospital where possible.
• Work across multiple locations and on a rota including nights, weekends, and bank holidays.
• Travel between sites and be willing to work outside normal hours as required.
• Carry out duties with safeguarding in mind and complete required DBS checks.
