Job summary

Leicestershire County Council seeks a role within the HART service to provide domiciliary care and reablement, supporting hospital discharge and independence. The job involves working across sites, with travel and on-call requirements, and DBS checks are essential.

The post requires NVQ level 2 (or equivalent) and experience across multiple agencies, along with strong communication and moving/handling abilities. A full UK driving licence and access to a vehicle are needed.

Qualifications

• NVQ level 2 or equivalent qualification in a relevant field or equivalent level of relevant experience.
• Experience of working with other agencies and/or in a multi-agency environment to identify and meet need with creative and pragmatic solutions.
• Ability to communicate clearly and accurately with a wide range of people, including representatives from other agencies, using negotiation and persuasion.

Responsibilities

• Provide homecare and reablement services to support timely discharge and independence.
• Coordinate with other agencies to identify needs and deliver solutions.
• Work to a rota including nights, weekends, and public holidays as required, with flexible hours.
