Job summary

Leicestershire County Council is seeking a dedicated HART Support Worker to deliver short, intensive reablement support for people in their own homes. You will help clients regain independence, assist with daily living tasks, and promote safe use of assistive technology.

Roles include working a rota between early morning and late evening hours, with weekends and bank holidays on occasion. DBS enhanced check is required.

Qualifications

• Hold a Level 2/3 Diploma in Health and Social Care (or equivalent)
• Be committed to delivering high standards of care and support
• Be able to work effectively on your own and as part of a team
• Have basic IT skills to write and maintain records
• Able to communicate effectively verbally and in writing and record observations
• Capable of working under pressure and remaining calm in crisis situations
• Be willing to attend job-related training as required

• Provide short, intensive reablement support to help clients regain independence
• Assist with daily living tasks and engage in community activities
• Identify suitable assistive technology and demonstrate safe use
• Offer strategies to manage fatigue and pain and prevent falls
