Job summary

Bosvena Health in Cornwall seeks an experienced GP with a special interest in frailty to join our Integrated Neighbourhood Team, supporting patients at home or in residential settings.

You will work within a multidisciplinary roster with community services, GP Practice leads, Frailty Matron, Occupational Therapists, nurses and pharmacy colleagues to manage complex older patients and prevent hospital admissions where appropriate. Flexible hours offered.

Qualifications

• Full and current GMC GP registration in the UK.
• Up-to-date knowledge of guidelines and evidence in older peoples medicine.
• Teaching qualification e.g. postgraduate diploma in medical education.
• Postgraduate qualification in older peoples care e.g. Diploma in Geriatric Medicine.
• Experience of EMIS, RIO and hospital clinical systems.
• High level of clinical skills and governance.
• Ability to supervise junior doctors and staff.
• Experience with a variety of clinical software systems.
• Experience of care planning and admission avoidance work.

Responsibilities

• Provide evidence-based, comprehensive medical care for older people.
• Work as part of a multidisciplinary team within holistic community pathways.
• Contribute to service development, local clinical protocols and pathways.
• Engage in research opportunities, audits and quality improvement.
