Job summary

Ash Trees Surgery in Carnforth, Lancashire, is seeking a Frailty and Urgent Care Advanced Clinical Practitioner to provide expert clinical leadership and advanced assessment for older adults with frailty and complex health needs, alongside urgent care duties within General Practice.

The post holder will work with our home visiting team and multidisciplinary colleagues to deliver frailty assessments, proactive care planning and same-day urgent care services, supporting admission avoidance and

• Experience in Frailty and Urgent Care.
• Ability to work collaboratively with multidisciplinary teams.
• Commitment to CPD and professional development.

Responsibilities

• Deliver advanced clinical assessment, diagnosis, treatment and management of frailty and urgent care needs.
• Independently manage same-day urgent care clinics and assess undifferentiated presentations.
• Undertake home visits, care home reviews and urgent community assessments.
• Coordinate care across primary care, community services and acute care.
• Provide leadership and support to the multidisciplinary team.
