Job summary

BrisDoc is seeking a Frailty-ACE Clinical Practitioner to work collaboratively within the Frailty-ACE to assess frail patients’ urgent care needs and coordinate holistic, person-centred management plans aligned with patients’ preferences and best interests.

The role covers a broad range of clinical presentations across age groups in urgent primary care, including coordinating referrals and maintaining sensitivity to capacity issues within the hosting acute trust.

Qualifications

• Experience working in urgent primary care settings is an asset.
• Strong communication and collaboration skills.
• Ability to manage sensitive patient information with empathy.

Responsibilities

• Assess frail patients' urgent care needs and coordinate management plans.
• Take telephone referrals from healthcare professionals in BNSSG area.
• Provide age-appropriate clinical assessments across physical and mental health domains.
