Provide Community seeks a Band 7 Nurse with non-medical prescribing qualification to join the Frailty Virtual Ward in Chelmsford. You will perform comprehensive geriatric assessments, develop management plans, and support patients in their own homes as part of a multidisciplinary team with UCRT collaboration.

Qualifications

• Extensive frailty knowledge and community care experience.
• Non-medical prescribing qualification.
• Assessment and consultation course completed.

Responsibilities

• Perform comprehensive geriatric assessments and develop personalised management plans with the MDT.
• Attend virtual GP calls and liaise with Frailty Consultant for complex cases.
• Lead skills development and discharge planning with the MDT to optimise length of stay.
