Job summary

Provide CIC is recruiting a Band 7 Nurse for the Frailty Virtual Ward in Chelmsford. The role involves comprehensive geriatric assessments, independent prescribing, and leading the MDT to optimise patient care in the community.

The post requires a minimum of 3 years of community experience, a non‑medical prescribing qualification, and a commitment to staff development and discharge planning. Working across 7 days with rotating shifts.

Qualifications

• Current registration with a professional body relevant to their profession.
• Qualified Independent Prescriber / willingness to attend training
• Management education development, including supervision/teaching/team management
• ACP accredited course

Responsibilities

• Undertake comprehensive geriatric assessments for patients under frailty virtual ward and formulate personalised management plans with the MDT.
• Attend rota-based virtual GP calls and liaise with Frailty Consultant for complex cases.
• Lead on developing staff skills, train juniors, support discharges with the MDT to optimise length of stay.
• Inspire others, challenge traditional practices and encourage innovative problem solving among staff.
• Agree outcomes for frailty patients with complex comorbidity to reduce hospital admissions.
• Develop innovative ways to improve flow in the frailty virtual ward with UCRT.
