Devon Partnership NHS Trust's FIND team in Community Forensic Services (CFS) is seeking a Mental Health or Learning Disability Nurse to join as a Senior Mental Health Practitioner. The post combines high-level assessment, treatment planning and case management for service users with complex forensic LD/ASC/ABI needs.

You will work within a multidisciplinary team, coordinating care, risk management and recovery-focused interventions while collaborating with a range of statutory and non-statutory

Qualifications

• NMC registration in Mental Health or Learning Disability
• Experience in forensic or community mental health settings
• Ability to manage complex caseload across multi-agency teams

Responsabilités

• Carry a case load with complex forensic LD/ASC/ABI.
• Provide expert assessment and treatment for patients with complex forensic LD/ASC/ABI.
• Deliver care management, planning, review and evaluation.
• Liaise with statutory and non-statutory agencies.
• Provide supervision and contribute to service development.
