Job summary

Leeds Community Healthcare NHS Trust is recruiting a Band 7 District Nurse to join the Yeadon Neighbourhood Team. Qualifications

• Must hold the Community Specialist Practitioner District Nurse course with V300 prescribing.
• Experience delivering care in patients' homes and community settings.
• Ability to lead a multidisciplinary team and manage complex caseloads.
• Willingness to work across the locality with a flexible roster.

Responsibilities

• Provide expert clinical care in patients' homes and community settings.
• Lead a cluster of Staff Nurses and Clinical Assistants to deliver high‑quality care.
• Mentor junior staff and support service development within the Yeadon Neighbourhood Team.
• Collaborate with GP practices and PCNs to reduce unnecessary hospital admissions.
