Job summary

NHS Greater Glasgow and Clyde (NHSGGC) is recruiting a District Nurse (Specialist Practitioner) to provide leadership and expert clinical care within the District Nursing Service for adults in community settings.

The post holder will oversee a multidisciplinary team, coordinate caseloads, and perform nurse prescribing within scope, including out-of-hours care. A live NMC pin is required.

Job description

The role:

The District Nurse (Specialist Practitioner) provides professional leadership and clinical expertise within the District Nursing Service, delivering high-quality, person-centred care to adults within community settings. The post holder is responsible for the assessment, planning, implementation and evaluation of complex nursing care, supporting patients to maintain independence and achieve the best possible health outcomes within their own homes.

As the caseload holder, the District Nurse leads and manages a multidisciplinary approach to care, working closely with patients, families, carers, social care services, GPs and other healthcare professionals to coordinate safe, effective and holistic care. The role involves managing patients with a range of complex health needs, including chronic disease management, palliative and end-of-life care, complex wound management, venous access devices, tracheostomy care and other specialist clinical interventions.

The post holder will provide clinical leadership to the District Nursing Team, supporting and supervising registered nurses, assistant practitioners and healthcare support workers, while ensuring the effective allocation of resources and delivery of services across a 24-hour, seven-day community nursing service. The role requires autonomous decision-making, advanced clinical assessment skills, risk management, caseload coordination and nurse prescribing within scope of practice.

Duration, Location, and Working Pattern:

• You will be based at Glasgow City HSCP
• Shift pattern for this psot are Mon-Sun various hours between 1700-21:30, weekend rota and public holiday cover
• Full Time Contract (36 hours per week)
• Permanent Contract

Key responsibilities:

• The post holder will provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies.
• The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to support and enable the district nursing team to provide a quality nursing service.
• The post holder will be a registered nurse with specialist district nursing qualification who supports in delivering care to patients within the caseload in Out of Hours period.
• The post holder will be responsible for providing skilled nursing interventions for patients within the caseload.
• The post holder will coordinate workload and delegate work to other staff, and provide support to other members of district nursing team.
• The District Nursing service operates an open access policy that is available 365-days a year. The service does not operate a waiting list; however, care needs are prioritised to be able to respond to both planned and unplanned care. This includes the ability to respond timeously to unscheduled care requests for those patients who require urgent district nursing intervention.
