Buckinghamshire Healthcare NHS Trust is recruiting an Advanced Community Practitioner (Band 7) for Thame ACHT. The role focuses on leading district nursing and complex case management within the community, driving patient-centred care across Thame and surrounding areas.

The post requires Specialist Practitioner Qualification in District Nursing and offers flexible working patterns and opportunities for leadership development within an integrated care system.

Qualifications

• Educated to Degree level or equivalent in health subjects.
• Registered Nurse with Level 1 registration.
• District Nursing SPQ and post graduate clinical training.
• Independent prescriber (non-medical) desirable.

Responsabilités

• Provide skilled assessment and complex case management.
• Lead and develop staff within a multi-disciplinary community team.
• Manage highly complex and unstable patient caseloads.
• Develop systems to support intensive case management with partners.
