Qualifications

Specialist Practitioner Qualification in District Nursing is essential.
Experience leading community nursing teams.
Ability to manage complex caseloads and multi-disciplinary teams.

Responsibilities

Provide skilled assessment of health needs and complex case management.
Proactively identify high intensity users of community and primary care.
Educate and support multi-disciplinary teams to intensively case manage identified patients.
Intensively manage own caseload of patients with highly complex needs.
Develop systems and processes to support intensive case management within the MDT and with partners across the health system.
Collaborate with other agencies to enable intensive management to prevent hospital admission and support early discharge.
