Job summary

Cornwall Partnership NHS Foundation Trust is seeking a Senior Community Mental Health Nurse (Band 6) in Redruth to deliver assessment, treatment and care coordination for people living with dementia and older adults with mental health needs. Based at Longreach House, you will travel to homes and care settings while collaborating with a multi-disciplinary team.

Qualifications

• Current professional registration – NMC.
• Post graduate qualification or equivalent experience in a psychological therapy e.g. CBT.
• Substantial experience of managing a busy caseload of clients with Dementia and a range of mental health problems and frailty in old age.

Responsibilities

• Provide assessment, treatment and care coordination to people living with dementia.
• Provide care to adults over 75 with mental health needs.
• Work as a senior team member and liaise with statutory and non-statutory agencies under the care programme approach.
