Job summary

Birmingham Community Healthcare NHS foundation Trust is seeking an enthusiastic counsellor to provide a counselling service for patients and families known to the Hospice, both pre and post bereavement, and for bereaved clients outside of the hospice.

The role supports the Service Lead with operational service efficiency, data management, colleague support, triaging and allocation, and involves assessing needs to plan and implement care while maintaining clinical records in line with hospice

Responsibilities

Provide a counselling service for patients and families known to the Hospice, both pre and post bereavement, and for bereaved clients outside the hospice.
Support the Service Lead with operational service efficiency, data management, colleague support, triaging and allocation.
Assess nature, severity and complexity of patients and families' requirements through assessment to formulate and implement a plan for their care.
Maintain clinical records in line with the Hospices policies and procedures.
