Job summary

JustOT is seeking an experienced Band 8a Complex Care Programme Case Manager to join a South East London service on a locum basis. You will lead complex care pathways across learning disabilities, autism and older people's services, ensuring person-centred planning and high-quality outcomes.

The role requires strong leadership, autonomous decision-making and the ability to coordinate multi-agency teams to deliver safe, effective care for individuals with complex needs in a dynamic environment.

Qualifications

• Experience leading complex care caseloads across LD/autism/older people's services.
• Ability to coordinate multi-disciplinary teams and external agencies.
• Strong communication and organisational skills.

Responsibilities

• Manage complex care cases across learning disabilities, autism and older people's services.
• Coordinate care planning and ensure effective management of complex needs.
• Work collaboratively with multidisciplinary teams and external agencies.
• Support the development and delivery of complex care programmes.
• Review care pathways and identify opportunities to improve outcomes and service delivery.
• Provide specialist advice, guidance and support to colleagues and partner services.
