Job summary

Sanctuary Personnel is seeking an experienced Occupational Therapist to join a Complex Care Programme Case Manager role in Lewisham, London. This locum post offers a pay rate of up to £37.00 per hour, with Monday to Friday 09:00–17:00, for an initial 2-month period with potential extension.

The role focuses on adults with learning disabilities, autism, and older adult needs, providing person-centred care within a multidisciplinary team and a supportive work environment.

Qualifications

• Degree in Occupational Therapy.
• HCPC Registration.
• Experience with learning disabilities, autism, and older adult care.
• Demonstrated ability in complex case management and care coordination.
• Experience in NHS Mental Health, community settings, or a case management qualification.

Responsibilities

• Manage complex care programmes for adults with learning disabilities, autism, and older adult needs.
• Coordinate care across health and social care services, ensuring a seamless delivery of support.
• Assess complex cases and develop personalised, person-centred care plans.
• Work collaboratively with a multidisciplinary team to ensure comprehensive care for service users.
• Support service users, carers, and families, providing guidance and assistance as needed.
• Coordinate interventions and monitor outcomes, making adjustments to care plans as needed.
• Maintain accurate records and documentation, ensuring compliance with relevant policies and procedures.
• Contribute to risk management and care planning processes, enhancing the safety and effectiveness of care.
