Job summary

Dorset HealthCare seeks an experienced Team Lead for the Intermediate Care and Rehabilitation Team (ICRT). You will provide clinical and operational leadership to a multidisciplinary team delivering person-centred rehabilitation in community settings, primary care and social care interfaces.

As part of the leadership, you’ll drive service development, ensure NICE guideline compliance, support staff through change, and work with partners to maximise patient independence and reduce unnecessary

Qualifications

• Must be a registered healthcare professional (RN/OT/PT).
• Demonstrated leadership and supervisory experience in multidisciplinary teams.
• Ability to lead MDT community rehabilitation and drive service development.

Responsibilities

• Provide clinical and operational leadership to a multidisciplinary team.
• Lead the delivery of rehabilitation interventions in community and integrated care settings.
• Ensure NICE guidelines are followed and performance targets are met.
• Promote collaboration with primary care, social care and secondary care.
• Support teams through change and develop partnerships with key stakeholders.
