Job summary

Dorset HealthCare University NHS Foundation Trust is seeking an experienced Team Lead for the Intermediate Care and Rehabilitation Team (ICRT) to drive community rehabilitation across locality services.

You will provide clinical and operational leadership to a multidisciplinary team, support patients to maximise independence and reduce hospital admissions, and work with partners across primary, community, acute and social care to deliver integrated care.

Qualifications

• Registered practitioner with post-registration specialist training or short courses.
• Membership of the relevant professional body and ongoing professional development.
• Evidence of recent development in an up-to-date portfolio.

Responsibilities

• Lead the MDT community team with a focus on rehabilitation and patient flow.
• Develop and maintain relationships with commissioners, GPs and social care colleagues.
• Provide clinical leadership for the community rehabilitation team and supervise staff.
• Ensure services meet NICE rehabilitation guidelines and key performance indicators.

Education

Registered practitioner to degree/diploma level with post-registration specialist training

Holds a valid UK driving licence

Membership of the relevant Professional Body

Evidence of recent professional development in up-to-date portfolio
