Job summary

Based in Thatcham, you will work within a multidisciplinary team to deliver high-quality community care, coordinate with social services, and contribute to integrated service development across Berkshire West.

Qualifications

• HCPC registration as a Physiotherapist and excellent communication skills.
• Understanding of intermediate care, rehabilitation and hospital discharge.
• Driver with a valid UK driving licence and flexible working days.

Responsibilities

• Support hospital discharge referrals to promote independence for on-going caseload.
• Support falls pathway with environment and clinical assessments to reduce fall risk at home.
• Provide equipment to promote independence and safety.
