Job summary

York and Scarborough Teaching Hospitals NHS Foundation Trust is seeking a qualified District Nurse or senior community nurse to join the Community Nursing team within the Integrated Care Service. You will manage caseloads, work in multidisciplinary settings, and provide leadership to support change while delivering safe, compassionate care across locality-based assessments and interventions.

Job description

An opportunity has arisen for a qualified District Nurse or an experienced Senior Community Nurse to work within the Community Nursing team of the Integrated Care Service

We are looking for a dynamic, passionate, innovative and committed community nurse with experience of caseload management who is either a qualified District Nurse. You will have skills in multidisciplinary working, together with excellent communication skills, resilience and great leadership skills. You will have the ability to motivate and support others through change in order to enable us to maintain the delivery of high quality, compassionate, proactive and safe care. The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy.
