Job summary

Procare Community Services in Milford seeks an experienced Community Nurse to join the Community Nursing team. The role focuses on unscheduled care to avoid hospital admissions and to provide a two‑hour response when clinically indicated.

The post offers Agenda for Change terms and NHS Pension, with opportunities for learning and development within the Royal Surrey NHS Foundation Trust ecosystem. Shifts cover 8am–8pm, Monday to Sunday, with driving licence requirements as applicable.

Qualifications

• Registered Level 1 Nurse with current Nursing and Midwipery Council registration.
• Practice Supervisor and Assessor (to be completed within 1yr of post)
• Band 6 experience in the community setting
• Ability to prioritise visits in accordance with patient need and to organise workload without direct supervision
• Experience prioritising, and allocating assessments in accordance with patient clinical need and individual competencies

Responsibilities

• Deliver nursing care in accordance with national and local policy and guidance.
• Ensure two hour Urgent Community Response when clinically indicated.
• Visit adult patients and provide high-quality community nursing care.
• Use an electronic patient-held record system and maintain accurate documentation.
