Responsabilités

• Provide care in Community/District Nursing within a defined geographical area.
• Hold responsibility for the clinical caseload and support the Integrated Team Manager in leadership.
• Lead and manage the nursing team and ensure quality and standard of care delivered.
• Support care home, carers and voluntary sector staff to promote independent living and avoid hospital admissions where appropriate.
