Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a passionate Registered Nurse (Adult / RGN) to join the new Home First service in Stroud. The role is part-time (18.75 hours per week) and focuses on supporting patients to return home safely from hospital, with a key role in care planning and multidisciplinary collaboration.

Qualifications

Registered Nurse (Adult / RGN) with current NMC registration.
Individual modules at degree or masters level, e.g. PACR.
Holds a recognised teaching/mentoring qualification.

Responsibilities

Perform holistic nursing assessments and develop personalised care plans for discharged patients.
Support safe and timely hospital discharges and prevent avoidable admissions.
Deliver evidence-based nursing care for patients with complex or long-term conditions.
Identify ongoing nursing needs and coordinate handover to Community Nursing Teams.
Promote independence and health coaching to empower patients.
