Job summary

Gloucestershire Health and Care NHS Foundation Trust in Gloucestershire is seeking committed Band 5 nurses with community experience to manage a patient caseload in home settings. You will work as part of the Severn Health Team in Stonehouse, providing holistic, patient-centred care and liaising with GPs and other agencies.

Newly qualified nurses are welcome. A full UK driving licence is required; post-registration experience is helpful but not essential.

Qualifications

• NMC registration (adult, physical health) required.
• Post-registration experience helpful but not essential.
• Full UK driving licence required.
• Ability to manage a patient caseload with autonomous practice.
• Excellent clinical and communication skills.

Responsibilities

• Assess patient and carer needs through strength-based assessment.
• Develop individual care plans with patients and carers.
• Regularly evaluate care plans and report clinical changes.
• Provide skilled nursing care interventions and advice to colleagues, patients and carers.
• Administer and monitor medications per policies.
• Contribute to workload prioritisation and manage a delegated caseload.
• Maintain accurate patient documentation on the electronic record system.
• Act as Named Nurse for a defined cohort when experienced.
• Provide clinical triage within centralised community nursing referral centres.
