Job summary

Tees, Esk and Wear Valleys NHS Foundation Trust is seeking a Senior Practitioner to deliver triage, assessments and risk management within a dynamic Integrated Community Team. You will lead with care coordination and deliver evidence-based interventions in the community while providing supervision to junior staff.

You will work with families, carers, and partner agencies to promote holistic wellbeing. A pro-active, person-centred approach and commitment to continuous improvement are essential.

Qualifications

• Current professional registration with an approved professional body.
• Evidence of Continuing Professional Development.
• 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.

Responsibilities

• Triage, complex assessments and risk management.
• Provide interventions and care coordination for adults with complex needs.
• Deliver supervision and coaching to junior colleagues and MDT members.
