Job summary

Sussex Partnership NHS Foundation Trust is seeking an experienced Mental Health Nurse, Occupational Therapist or Social Worker to join our Community Mental Health Team as a Lead Practitioner.

The role involves working within a multidisciplinary team to assess, coordinate care, manage risk and deliver recovery-focused interventions that help adults with complex mental health needs to live fulfilling lives in their communities.

Qualifications

• Experience in community mental health settings.
• Ability to work within multidisciplinary teams.
• Competence in delivering risk assessments and care planning.

Responsibilities

• Manage a caseload of service users with complex mental health needs.
• Undertake comprehensive mental health and risk assessments.
• Develop, implement and review recovery-focused care plans.
• Act as Care Coordinator and Lead Practitioner in CPA processes.
• Deliver evidence-based interventions and specialist mental health advice.
• Work collaboratively with service users, carers, families and partner agencies.
• Provide supervision and support to students and junior staff.
• Maintain high-quality clinical records and contribute to service quality improvement activities.
• Contribute to multidisciplinary team discussions and care planning.
