Job summary

The CNTW NHS Foundation Trust is recruiting a Band 6 Community Practitioner to work within the South Tyneside Community Treatment Team, focusing on assessment and treatment for clients with moderate to severe mental health difficulties. You will join a multidisciplinary team and deliver NICE-guideline based care, with opportunities for supervision and professional development.

Must have a professional qualification in health or social care.
Registered with the relevant professional body.
Commitment to NICE guidance and high-quality care.

Responsibilities

Providing MDT assessment of clients' needs and identifying collaborative treatment plans.
Deliver various evidence based psychological therapies both individually and in a group setting.
Support the wider team in ensuring the provision of excellent standards of care.
Have a good understanding of pharmacological approaches to treatment.
To work together with clients and their families through the journey of care.
To maintain effective working relationships with other CNTW services and external agencies.
To support with the implementation of Community Transformation agenda.
Working collectively with the team in maintaining individual and team wellbeing.
