Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking a Band 6 Community Practitioner in Jarrow to join the psychosis pathway. You will work within a multidisciplinary team, delivering assessments and treatments for clients with moderate to severe mental health needs, and provide leadership to the team.

Qualifications

• Professional qualification and current registration as RNMH/RMN/RNLD or equivalent in an AHP, Social Worker or Clinical Psychologist
• Post-registration training in clinical/psychological interventions
• Proven post-registration clinical experience in community settings
• Exposure to CPA, Safeguarding, MAPPA and Child Protection
• Up-to-date knowledge of relevant legislation and Post Registration qualifications in Psychological Interventions
• Experience in working within a community team
• Non-Medical Prescribing where applicable

Responsabilités

• Provide MDT assessment of clients’ needs and develop collaborative treatment plans
• Deliver evidence-based psychological therapies individually and in groups
• Support the team to maintain high standards of care
• Understand pharmacological approaches to treatment
• Collaborate with clients and families throughout care journeys
• Engage in ongoing training for professional development
• Maintain relationships with CNTW services and external agencies
