Job summary

OneMedical Group is seeking a Registered Nurse with V300 Independent Provider status to join our Community Matron team in Minehead. You will build trusting relationships with patients and families, providing proactive frailty-focused care for older adults in the community.

Based at Minehead Medical Centre, you will deliver holistic care across diverse conditions, carry out polypharmacy reviews, support anticipatory care planning, and help people stay independent and avoid unnecessary hospital

Qualifications

• Registered Nurse with full qualification and V300 Independent Provider status.
• Experience or interest in frailty, geriatrics, and caring for older adults.
• Ability to provide proactive clinical care and perform polypharmacy reviews.

Responsibilities

• Support patients of all ages with complex health needs in the community.
• Deliver holistic, person-centred care with a focus on frailty.
• Develop anticipatory care plans and help patients remain independent at home.
