Job summary

Procare Community Services is seeking a Community Matron to lead clinical care for adults with complex long‑term conditions. You will manage a caseload, perform advanced assessments, and coordinate care with GPs, social care, and mental health teams to keep people at home and out of hospital.

Qualifications

• Registered Level 1 Nurse with current NMC registration.
• MSc or Level 7 qualification with health assessment knowledge.
• Full UK driving licence and vehicle for business use if required.

Responsibilities

• Provide expert clinical leadership and proactive case management for adults with complex, long-term conditions.
• Coordinate care across the Integrated Neighbourhood Team and partner organisations.
• Work to prevent avoidable hospital admissions and support independence at home.
• Collaborate with GPs, social care, mental health teams and other partners.
• Deliver comprehensive assessments and develop personalised care plans to improve outcomes.
