Job summary
Berkshire Healthcare NHS Foundation Trust is seeking a Community Staff Nurse to join the West Berkshire Community Nursing team. You will support, assess and treat patients in their own homes, providing high-quality, evidence-based care seven days a week, 08.30–19.00, including Bank Holidays.
You will work with the Team Leader/Community Nursing Band 6/Matron and other professionals to holistically plan and evaluate nursing care, teaching patients and families and promoting self-care where
Qualifications
- Assessment, reassessment, and medication management.
- Bladder and bowel care and tissue viability.
- Care using specialist equipment and palliative care.
- Commitment to holistic home-based care.
Responsibilities
- Holistically assess the needs of patients and carers and plan care with the patient and family.
- Undertake first visits and report back to the Team Leader/Community Matron for further discussion.
- Establish working relationships with other professionals and voluntary agencies.
- Provide care according to best practice and support evidence-based development.
- Promote self-care and build health promotion into care.
- Develop and participate in teaching programmes for patients.
