Job summary

Central East Integrated Care Board in the UK is seeking a CYP Continuing Care Assessor (Band 7) to lead complex assessments for children and young people with significant ongoing health needs. You will ensure robust, defensible recommendations aligned with the CYP Continuing Care Framework and work across families, local authorities, education, and health services.

Qualifications

• Experience in leading complex assessments for CYP with significant ongoing health needs.
• Ability to analyse multidisciplinary evidence and provide defensible recommendations.
• Strong communication skills with families, partners, and safeguarding teams.

Responsibilities

• Undertake complex holistic assessments for CYP under Continuing Care.
• Analyse MDT evidence and provide clear eligibility recommendations.
• Lead MDT discussions and case reviews with senior clinical input.
• Explain processes to families with sensitivity and clarity.
• Liaise with local authorities, SEND, education, safeguarding, and health providers.
• Develop and quality-assure support plans aligned with assessed needs.
• Manage a caseload of complex cases with timely progression and governance.
• Provide senior clinical advice and mentorship to colleagues.
• Identify safeguarding risks and escalate where needed.
• Contribute to audit, quality monitoring and pathway improvements.
• Maintain accurate records and ensure information governance.
