Central East ICB is seeking a Band 6 CYP Continuing Care Assessor to carry out high-quality assessments for children and young people with complex health needs. You will work with families and a multidisciplinary team to gather evidence and produce defensible recommendations that support safe, person-centred care planning.

The role requires strong assessment skills, professional judgement and excellent communication.

Qualificações

• Educated to degree level or equivalent in a relevant field.
• Registered with the NMC (NMC Registered).
• Ability to manage priorities and deliver to timescales.
• Experience coordinating health and social care settings is desirable.

Responsabilidades

• Undertake holistic assessments of children and young people with complex health needs using the CYP Continuing Care Framework.
• Gather, review and interpret evidence from health, social care, education and families.
• Develop clear, person-centred support plans reflecting assessed needs.
• Manage a defined caseload with timely assessments and documentation.
• Collaborate with families and MDT partners to coordinate evidence and planning.
• Escalate safeguarding concerns or high-risk cases as required.
