University Hospitals of North Midlands NHS Trust is seeking a registered Pharmacy Technician to join the Frontline team at Royal Stoke University Hospital in Stoke-on-Trent. This maternity cover post offers development within a dynamic clinical pharmacy service and the opportunity to contribute to EPMA implementation.

The role includes medication histories, medicines reconciliation, ward-based prescribing support and discharge planning, with emphasis on patient safety and collaborative working

Qualificações

• BTEC Level 3 Diploma in the Principles and Practice for Pharmacy Technicians or equivalent nationally recognised qualification
• Accredited accuracy checker
• Registered with the General Pharmaceutical Council

Responsabilidades

• Undertake medication histories and medicines reconciliation, prioritising patients on time critical medications, patients with medicine related admissions and patients that have been in A&E for a long period of time
• Assessing patients' own medicines
• Ordering non-stock time critical medicines using e-ordering system
• Undertake accuracy checking of Take Home Prescriptions and critical medicines
• Gain consent for the discharge medicines service process (DMS)
• Assist with relevant audits (Controlled drugs, fridge monitoring, excursion issues)
• Assist with clinical incident management and implementing actions to prevent re-occurrence
• Coordinate discharge planning with pharmacy and ward staff
