University Hospitals of North Midlands NHS Trust in Stoke-on-Trent is seeking a Registered Pharmacy Technician for maternity cover in the Emergency Medicine team. Band 5, 37.5 hours per week, fixed-term 12 months, with late shifts and weekends as part of a 4-week rota.

The role involves medication histories, reconciliation, and supporting the EPMA implementation, with opportunities for line management and leadership development within the NHS.

Qualifications

• One year post-qualification experience in an acute hospital setting.
• Experience undertaking medicine reconciliation.
• Experience of POD or equivalent schemes.
• Experience of undertaking audits.

Responsabilités

• Undertake medication histories and medicines reconciliation, prioritising time-critical medications.
• Assessing patients' own medicines.
• Order non-stock time-critical medicines using electronic ordering system.
• Undertake accuracy checking of Take Home Prescriptions and critical medicines.
• Gain consent for discharge medicines service (DMS).
• Assist with audits (Controlled drugs, fridge monitoring).
• Assist with clinical incident management and action planning to prevent recurrence.
• Coordinate discharge planning with pharmacy and ward staff.
