Northern Lincolnshire and Goole NHS Foundation Trust is seeking an experienced, forward-thinking clinical pharmacist at Band 8a to join us, combining advanced patient-facing practice with leadership in medicines safety, governance and service improvement. The post is split 0.6 WTE for surgical and critical care inpatient services and patient flow, with 0.4 WTE focused on PGDs and staff support.

Responsabilidades

• Provide direct patient care and support safe, effective clinical treatment across surgery, critical care and patient flow.
• Optimise medicines use by guiding medical and non-medical prescribing practice and, where appropriate, independently prescribing or deprescribing.
• Apply local, regional and national guidance, demonstrating excellent antimicrobial stewardship.
• Use sound clinical judgement and evidence-based practice to manage medicines shortages that affect patient care.
• Write and contribute to Standard Operating Procedures, policies and guidelines, identifying and addressing gaps in local practice.
• Recognise medicines-related incidents and near misses, support timely resolution, and signpost concerns appropriately.
• Provide expert formulary advice, review clinical guidelines and lead the review of PGDs for the safe supply or administration of prescription-only medicines in line with locally agreed policy.
• You will support the Pharmacist Clinical Team Manager in the delivery of the Pharmacy Vision and Values Strategy together with the other Clinical Pharmacy Team Leaders.
